By submitting this form, you are applying to become a Polistic Pole Yoga Ambassador and to receive a unique promo code that allows you to earn commission when your community joins or shops through your code.
To keep your promo code active, you agree to:
◆ Maintain an active Polistic+ Gold or Platinum membership under the email address you provide in this application.
◆ Share your code in an honest, non misleading way that reflects the spirit and values of Polistic Pole Yoga.
◆ Not use your code on spammy sites, coupon or deal sites, or in any way that feels deceptive or misleading.
◆ Not run paid ads on Polistic brand terms without written permission from Polistic Pole Yoga.
◆ Receive payouts via Cash App, PayPal, or other payout details you provide in this form, on a monthly payout schedule, on your earned commission.
◆ Understand that Polistic Pole Yoga may pause or deactivate promo codes that have no qualifying sales for 90 days, or in cases of suspected misuse, fraud, or violation of these terms.
◆ Understand that commission rates, payout thresholds, and program terms may change over time; any updates will be emailed to the address on your Polistic+ account.
